Description

This document is a bilingual sample of a Living Will (หนังสือแสดงเจตนาไม่ประสงค์จะรับบริการสาธารณสุข), drafted in accordance with Section 12 of Thailand's National Health Act.

A Living Will is intended for use by foreigners residing in or visiting Thailand who wish to formally express their medical treatment preferences in situations where they may be unable to communicate. The Living Will allows individuals to refuse life-prolonging treatments under certain medical conditions, and is recognized under Thai law when properly completed. This PDF version is provided in both Thai and English to support clear understanding and communication between patients, medical professionals, and caregivers.
